The Platform is owned and operated by SimpleQI, LLC ("SimpleQI" "we" "us" "our"). This Privacy Policy may be updated from time to time for any reason. SimpleQI provides a pop-up or notice on the Platform whenever updates to this policy are made. It is the responsibility of all users of the Platform, however, to revisit this page to check for changes. Please read this Privacy Policy carefully before accessing or using the Platform. By accessing or using any part of the Platform, you agree to become bound by the terms and conditions of this Agreement. If you do not agree to this Privacy Policy, then you may not access the Platform or use any services.
By using simpleqi.com you are consenting to our processing of your personal data as set forth in this Privacy Policy now and as amended by us. “Processing” means using cookies on a computer or using or touching information in any way, including, but not limited to, collecting, storing, deleting, using, combining, and disclosing information, all of which activities will take place in the United States. If you reside outside the U.S. (unless otherwise agreed via a written contract) your personal data will be transferred to the U.S., and processed and stored there under U.S. privacy standards. You will be asked to provide, your name, project and team name and email. In communicating with us, you will also have an opportunity to share additional information with SimpleQI, in your sole discretion, which may include personal data. This information is seen only by SimpleQI and is not publicly available. Use discretion when sharing information through chat functions, emails, and contact forms.
You have the right to obtain our confirmation of whether we maintain personal data relating to you. You may access, review, update, erase, or block any information in your account on the Platform, as permitted by applicable law and SimpleQI policies.
Deletion or removal of certain information may reduce functionality of the Platform or prohibit you from engaging in certain activities on the Platform or certain SimpleQI services, where such information is required to provide those activities and services.

SimpleQI uses non-personal data to monitor the use and performance of the Platform so that SimpleQI can learn more about how people use the Platform which helps us improve SimpleQI’s services and significantly enhance your experience on the SimpleQI Platform.
Log Data.
When you visit the Platform, our servers automatically record information that your browser sends whenever you visit a Platform (“Log Data”). This Log Data may include information such as your IP address, browser type or the domain from which you are visiting, your geographic location, the pages you visit, length of each page visit, and/or functionality you use/activities on the Platform that you participate in. For most users accessing the Internet from an Internet service provider, your IP address will be different every time you visit the Platform. We use aggregate and anonymized Log Data to monitor the use of the Platform and of our service, and for the Platform’s technical administration. We do not associate your IP address with any other personal data to identify you personally, except as required to enforce this Privacy Policy and the Terms of Service for the Platform, or as necessary to protect the rights, property, or safety of SimpleQI, its employees, customers, or other users.
Personal data Accessible by Other Users.
By registering an account on the Platform and engaging in activity on the Platform, you authorize your personal data in the form of your name and email, to be made available to other users on the Platform within any project that you are a member.
SimpleQI provides functionality allowing communications between users on the Platform. SimpleQI has the right, but not the obligation to monitor your communications and further SimpleQI cannot control the content of those communications or how a user will make use of such content. Use discretion when sharing content, personal information, or otherwise communicating with other users. SimpleQI provides functionality allowing you to create custom forms and questionnaires to be distributed and completed by those users participating in a particular project. Information you share through custom created forms, questionnaires, or any communications within the Platform can be seen by other users participating in your project team.
Use discretion when sharing content within your account and your project team. We may, from time-to-time monitor or review the communications sent through our Platform messaging tools, so that we are able to identify potential issues and implement more efficient solutions for provision of our services, as well as to make the Platform more user-friendly and undertake appropriate measures if the users do not conduct themselves with our Terms of Service.
Data Accessible by Other Organizations.
In the interest of transparency, any data used, uploaded, or shared within the Platform, associated with a particular project, is viewable by all project members, including users from other organizations. Use discretion when sharing content within your account and your project team.
Cookies.
SimpleQI uses essential cookies on the Platform in order to improve the quality of the Platform and services to you. A cookie is a string of information that a Platform stores on a user’s computer and the user’s browser provides to the Platform each time the user returns. SimpleQI uses cookies to help SimpleQI identify and track users, their usage of the SimpleQI Platform, and their Platform access preferences. Cookies do not collect or store personal data. Rather, the cookie stores a token which uniquely identifies a particular user at a particular time. That token is then linked to the user information in SimpleQI’s secured databases. SimpleQI users who do not wish to have cookies placed on their computers may disable cookies from their account and additionally, users can set their browsers to refuse cookies before using the Platform, with the drawback that certain features of the Platform may not be available to you or may not function properly without the aid of cookies.

Do Not Track (“DNT”) is a privacy preference that users can set in certain web browsers. DNT is a way for users to inform websites and services that they do not want certain information about their webpage visits collected over time and across websites or online services. Certain state laws require us to indicate whether we honor DNT settings in your browser.
SimpleQI adheres to the standards set out in this Privacy Policy and does not recognize, monitor, follow, or respond to browser-initiated DNT signals.
For your convenience, we have provided you with some opt-out options that you can exercise, below. These links may not represent all opt-out options available or third parties collecting information via the Platform.
Your California Privacy Rights:
In addition to other rights provided for herein, if you live in California and have an established business relationship with us, you can request a list of the personal data we have shared with third parties for their marketing purposes. We will also give you a list of the third parties that have received your information. You can make a request one time each year. To exercise your rights, you can email us or write to us at the address below. Mention in your letter that you are making a "California Shine the Light" inquiry. We will respond within 30 days.
The California Consumer Privacy Act (“CCPA”) provides consumers with specific rights regarding their personal data. You may also request that we provide you with an accounting of your personal data held by SimpleQI over the past twelve (12) months. You may also request that SimpleQI delete your personal data, subject to certain exceptions. If SimpleQI were to sell your personal data, you have a right to opt-out of that sale. You may submit that request to via email at support@simpleqi.com, or via mail 16 Applewood Road, Norfolk, MA 02056. Upon verification of your identity and within 45 days, we will provide you with a paper copy of your personal data via the United States Postal Service. SimpleQI will not discriminate against any user for exercising their rights under the CCPA.
SimpleQI does not sell your personal data, as those terms are defined under the California Consumer Privacy Act.
Information Sharing and Disclosure.
Service Providers. We engage certain trusted third parties to perform functions and provide services to us, including, without limitation, hosting and maintenance, customer relationship management, database storage and management, and direct marketing campaigns. We will share your personal data with these third parties, but only to the extent necessary to perform these functions and provide such services, and only pursuant to binding contractual obligations requiring such third parties to maintain the privacy and security of your data, by (i) agreeing to provide adequate protections for personal data that are no less protective than those set out in this Privacy Policy; and (ii) notifying SimpleQI in the event that such third party is no longer able to provide sufficient protections.

For users located within the EU, UK, or Switzerland, pursuant to GDPR, SimpleQI remains liable for the transfer of personal data to third parties acting as our agents unless we can prove we were not a party to the events giving rise to the damages.

SimpleQI believes in the privacy of our Platform users. We safeguard the confidentiality of your personal data. We employ administrative, physical, and electronic measures designed to protect your information from unauthorized access. We will make any legally-required disclosures of any breach of security, confidentiality, or integrity of your unencrypted electronically-stored personal data to you via email or conspicuous posting on this Platform in the most expedient time possible and without unreasonable delay, consistent with (i) the legitimate needs of government authorities, including to meet national security or law enforcement requirements or (ii) any measures necessary to determine the scope of the breach and restore the reasonable integrity of the data system.
How does SimpleQI secure and store your information?
The security of your personal data is important to us. SimpleQI uses an SSL Protocol to secure the Platform. SimpleQI employs industry standard precautions designed to protect your information from unauthorized access. SimpleQI ensures that your data is encrypted at rest. SimpleQI will make any legally-required disclosures of any breach of the security, confidentiality, or integrity of your unencrypted electronically stored personal data to you via email or conspicuous posting on this Platform in the most expedient time possible and without unreasonable delay, consistent with (i) the legitimate needs of law enforcement or (ii) any measures necessary to determine the scope of the breach and restore the reasonable integrity of the data system. Remember that no method of transmission over the internet or method of electronic storage is 100% secure. We cannot promise, and you should not expect, that your personal data or private communications will always remain private. SimpleQI cannot guarantee complete security. SimpleQI currently stores user information via secure cloud-based web hosting services, provided by Microsoft Azure and the information is stored on servers located within the United States.
Retention of Data
SimpleQI retains personal data when we have an ongoing legitimate business, legal, or compliance need to do so. Our retention periods will vary depending on the type of personal data. The general factors we consider in determining whether to retain personal data are:
When SimpleQI has no ongoing legitimate business need to process your personal data, we will either delete or anonymize it or, if this is not possible (for example, when personal data has been stored in backup archives), then SimpleQI will securely store such personal data until deletion of such information is possible.

The Platform and communications sent between users or between users and SimpleQI may contain links or references to other websites. SimpleQI cannot control and cannot be held responsible for the content that appears on those sites, and is not an endorsement, authorization, or representation of our affiliation with that third party. Furthermore, SimpleQI is not responsible for the privacy policies of any third-party website or for how they treat information about their users. These third-party sites may place their own cookies on your computer, collect data, solicit personal data, or sell personal data. Our Policy Towards Children.
The Platform is not directed to persons under 18. We do not knowingly collect personal data from users who are considered children. If a parent or guardian becomes aware that his or her child has provided us with personal data without their consent, he or she should contact us at support@simpleqi.com. We will take actions to promptly remove such data. We do not knowingly collect personal data from children under 18. If we become aware that a child under 18 has provided us with personal data, we will delete such information from our files.
Use of this Platform by individuals located in the EU, UK, and Switzerland
If you are a resident of the European Union, United Kingdom, or Switzerland or provide personal data through this Platform while located in the European Union, United Kingdom, or Switzerland, we offer you certain choices in connection with the personal data we collect from you on this Platform.
